Health Assessments for NY CDPAP PPL Personal Assistants

Welcome CDPAP Personal Assistants (PAs)

The IMA Group is a clinical evaluations and assessment services company contracted by PPL to conduct required health assessments for New York CDPAP Personal Assistants. Our role is to help PAs meet the New York State Department of Health regulations to start or continue their employment as a caregiver.

Health Assessment Options

New Personal Assistants

If you have registered with PPL and The IMA Group has been assigned as your health assessment provider, you will receive a link from us to schedule your pre-employment health assessment. The assessment is free when completed through The IMA Group.

If you are interested in becoming a personal assistant and have not yet registered with PPL, please contact PPL to initiate the registration process. Click the link below for more information.

Health Assessments and Medical Documents

Health assessments are required to work in CDPAP and must be completed on an annual basis. The assessment includes a physical exam, TB test, and proof of immunity for Measles and Rubella.

If you submitted medical documents to PPL, we are reviewing them and will contact you with next steps.

To complete your health assessment with your own provider/PCP, download this form and bring it to your appointment.

Annual Compliance

We’ll email you — it’s that simple. Two quick forms, once a year — about 10 minutes total.

  1. A Self Health Assessment
  2. A Tuberculosis Risk Assessment or Survey
  3. Email Reminder: The IMA Group will email you links to both forms up to 30 days before they are due. Example: renewal in July → email as early as June 1.

DON’T DELAY! Both forms must be completed and submitted in order to continue working as a PA in the NY CDPAP.

Follow-Up May Be Needed: Based on the medical review of your forms, you may need follow-up services. If so, The IMA Group will contact you to schedule it.
